You're Invited To C-TAC's National Summit on Advanced Illness!

Each year, the Coalition to Transform Advanced Care (C-TAC) hosts the National Summit on Advanced Illness Care, bringing together hundreds of healthcare leaders, policy makers, consumer advocates, academics and others in a new movement about comprehensive, value- and patient-focused care for people with advanced illness. This year’s Summit will be held at the Denver Hilton City Center in Denver, Colorado (October 9–11).

C-TAC recognizes that the important work of healthcare reform cannot happen solely in Washington, DC. It needs to occur on the ground with people interacting with their clinicians, local health providers, and community institutions. As part of our long-term commitment to working with our partners in Colorado, we would like to extend members of locally based organizations an invitation to attend the National Summit at a specially discounted rate ($200).

This year's summit will focus on how to implement innovative care and payment reform programs for people living with advanced illness. This event brings together over 400 healthcare stakeholders representing providers, health systems, health insurers, consumer groups, patient advocates, private-sector employers, and thought leaders from around the country. It’s fully interactive, providing ample opportunity to engage in meaningful dialog with presenters, meeting and sharing experiences with colleagues from around the country and exploring new and challenging approaches that can be immediately useful.

HPCAR/COCHPC: Slogan Contest

Submit a slogan for our license plate under consideration!

Please help us develop an impactful slogan that will be on the group license plates we are working to establish through the Colorado Department of Revenue. Your efforts will be recognized when the new slogan is announced in October.

The slogan should be 3 to 5 words long. The slogan should resonate with all Coloradans and reflect the value of life and health.

About our group license plate:
The Colorado Center for Hospice and Palliative Care, a 501c3 affiliate of the Hospice and Palliative Care Association of the Rockies, is working to establish a special group license plate.

Proceeds from the sale of these license plates will be used to provide education programs for hospice and palliative care organizations and for consumers as well as to promote the benefits of and provide information on hospice and palliative care to Colorado communities.

Part of our application to the Department of Revenue is to submit the artwork for the license plate, including a slogan.

This Hospice Compare update reflects Hospice Item Set (HIS) quality measure results based on data collected Q4 2016 – Q3 2017 and on Hospice Consumer Assessment of Healthcare Providers and Systems (CAHPS®) Survey ® results reported Q4 2015 – Q3 2017.
